Insurance Services Disclosures

By signing below, I’m confirming I understand all information and my responsibility as a client of Brighterday Insurance Services, LLC.

1) I understand that it is my responsibility to review the summary of benefits on the insurance plan that is presented to me and ensure I save a copy for the year it is eligible to refer to on all coverage and cost.

2) If I’m on an HMO plan, I understand that in most cases with HMO insurance plans I will need a referral from my primary care physician to any specialist. I also understand I cannot see a specialist until said referral is received by the specialist office or I could be charged in full for the doctor's visit.

3) It is my responsibility to check if my current providers are in my plan's network even though it’s been checked online in the carrier portal. Not all providers update carriers of changes and/or renew yearly contracts.

4) I am aware I need to check the formulary on my insurance plan to ensure my medications will be covered.

5) I understand that if I move I need to inform my agent within 2 weeks as plans could change based on changes of counties and states. It is best to communicate before making the move to know what to expect for your new area. I understand moving is a qualifying life event and I’ve got 60 days to make the change or my plan could be terminated for being outside the service area.

6) It is my responsibility to ensure my premium payments are made on time per month and confirm automatic payments are being deducted. You can work directly with member services to verify your account is current. If your plan is terminated due to lack of payment you may have to wait until open enrollment to get a new insurance plan. 

7) It is my responsibility to update my agent throughout the year of the following changes: Income & Family Size (amount of dependents).

8) It is my responsibility to follow all heathcare.gov guidelines such as filing jointly if married & filing taxes annually. I understand I could pay back my full tax subsidy if taxes are not filed accordingly and my plan could be automatically cancelled if I do not file my taxes within the same insurance year.

By signing below, I acknowledge that my licensed insurance agent and/or their agency may receive compensation from the insurance company for assisting with my enrollment. Compensation may range from $0 to $28 per person per month, depending on the state and insurance carrier. Please note that certain insurance companies have chosen not to compensate agents for servicing their plans. As a result, our agency is unable to offer coverage or ongoing service for those specific plans in affected areas.

If you would like more details about the compensation received for a specific plan, you may contact our office. We will research and provide the most current information available; however, commission structures are changing frequently (sometimes weekly) and generally not in favor of agents, so exact amounts are not listed here.
